RECORDS

Having a copy of your previous records helps us treat you without, many times, having to repeat studies or labs.

IF ANY ONE OF THE FOLLOWING APPLY TO YOU, PLEASE BRING A COPY OF THE RECORDS (RESULTS) WITH YOU TO THE CLINIC.

• You have been recently discharged from a hospital or other medical facility
• You have seen another cardiovascular health care provider
• You have seen another health care provider for cardiovascular related tests (lab draws, stress tests)

PLEASE BRING A COPY OF THE RECORDS (RESULTS) WITH YOU TO THE CLINIC.

If you do not already have a copy - you can call the facility where the test(s) were performed, and speak to the "medical records" department. They will give you instructions on how to obtain your records. Most times, if possible, this is best accomplished in person.

IF POSSIBLE, PLEASE BRING A COPY OF YOUR INSURANCE INFORMATION.
